Experience in disability or home care services (or willing to train)
Hold a valid driver’s license and have reliable transportation
Assist clients with finding and maintaining housing, completing applications, and accessing community resources.
Provide one-on-one support to individuals in their homes or community to help them achieve daily living goals.
Deliver person-centered 245D waiver services according to each client’s individualized support plan.
