Supporting Stability, One Home at a Time

At WellHealth Services, we believe that stable housing is the first step to a more independent, fulfilling life. Our Housing Stabilization Services (HSS) are designed to help individuals with disabilities or mental health conditions find and maintain safe, long-term housing—with hands-on, compassionate support.

What is Housing Stabilization Services?

Housing Stabilization Services (HSS) is a Minnesota Medical Assistance (MA) benefit that supports people with disabilities, mental health needs, or other complex challenges in finding and keeping housing. It is not a rental assistance program—but a supportive service to remove housing barriers and promote long-term success in the community.

Who Qualifies?

To qualify for HSS, individuals must meet the following requirements:

  • 18 years or older

  • Enrolled in Medical Assistance (MA)

  • Have a documented disability or mental health diagnosis

  • Facing housing instability or risk of homelessness

  • Able to complete a professional statement of need and housing assessment

Not sure if you or someone you support qualifies? We’re happy to help you find out.
