Housing Stabilization Services (HSS) is a Minnesota Medical Assistance (MA) benefit that supports people with disabilities, mental health needs, or other complex challenges in finding and keeping housing. It is not a rental assistance program—but a supportive service to remove housing barriers and promote long-term success in the community.
To qualify for HSS, individuals must meet the following requirements:
18 years or older
Enrolled in Medical Assistance (MA)
Have a documented disability or mental health diagnosis
Facing housing instability or risk of homelessness
Able to complete a professional statement of need and housing assessment

We provide three core services under HSS
For individuals not already connected to a waiver or targeted case management service.
We help with:
Assessing housing needs and preferences
Developing a person-centered housing plan
Coordinating the referral and documentation process
For clients ready to find a new place to live.
We assist with:
Searching for safe and affordable housing
Completing applications
Gathering required documents
Communicating with landlords and housing providers
For individuals who have secured housing and need support to maintain it.
We provide help with:
Understanding lease responsibilities
Communicating with landlords
Budgeting and money management
Connecting with community services
Crisis planning and problem-solving
Compassionate, client-centered care
Local expertise in housing support
Quick, responsive communication
Culturally respectful, trauma-informed approach
Strong collaboration with case managers and families
Referring someone to our services is simple, secure, and the first step to helping them get the care and support they deserve.
